God's Princess Dance Camp (Ages 4-6)

By checking the box and completing my purchase, I acknowledge and agree to the following:

1. Assumption of Risk & Liability Waiver

I understand that participation in ROOTS Academy programs, including music, dance, and theater activities, involves physical movement that carries inherent risks of injury. I voluntarily assume all risks and release ROOTS Academy, its owners, employees, instructors, and affiliates from any claims, liabilities, or damages arising from participation, whether on-site or off-site.

I agree to indemnify and hold ROOTS Academy harmless from any claims, costs, or damages related to my or my child's participation. In case of an emergency, I authorize ROOTS Academy to seek medical treatment for my child if I cannot be reached, understanding that I am responsible for any associated medical costs.

2. Refund & Credit Policy

• Cancellations at least one month before the event start date are eligible for a refund in the original form of payment, minus a $50 processing fee.
• Cancellations within one month of the event start date will receive a 50% refund in the original form of payment.
• Once the event begins, fees are non-refundable.
• ROOTS Academy does not issue refunds for missed classes due to illness, vacations, weather, or other personal reasons.

3. Event Cancellation Policy

ROOTS Academy reserves the right to cancel any event that does not meet the minimum enrollment requirements. Any cancellation decision will be made at least two weeks before the event start date, and a full refund will be issued.

4. Photo & Video Release

By completing this purchase, I grant ROOTS Academy permission to capture and use my child's image, video, and name in promotional materials, including social media and marketing.

To opt out, I will email dance@rootsacademy.com before the event begins.

5. Acknowledgment

By checking the box at checkout and purchasing this event, I confirm that I have read, understand, and agree to these terms.
